Death Certificates:

To obtain a death certificate in our Health Department, the individual must have died in Jackson County.

Obtain Through Mail

  

To apply by mail, print the above application and send the following information:

 

Your Name

Address

Daytime Telephone Number

Date

 

Name of Deceased

Date of Death

Place of Death

Reason Needed

Enclose a money order payable to Jackson County Health Department. Certified Death Certificates are $15.00 each

Mail to: Jackson County Health Department  801 W 2nd St., Seymour  IN  47274

Enclose a stamped, self addressed envelope.
